The following information will only be used to ensure that the NCT course facilitator can take full account of your needs during the course. It will not be shared with any other course participants (unless you choose to). If you do not want to give the information here you will have the opportunity to discuss the matter with the NCT course facilitator.

Please add any additional information that would be helpful for class facilitator to know, e.g. state of health, any special medical conditions, allergies, special needs, personal circumstances etc.
